Over recent years social prescribing (SP) has emerged as an effective way of providing GPs with a non-medical referral option that can operate alongside existing treatments to improve patient health and well-being.

At a personal level, changes in lifestyle can significantly reduce your risk of developing or exacerbating LTC. At both a local and national level, evidence from community health interventions demonstrate that a contributory factor to health and wellbeing is being linked into a local community and reduced social isolation.

This discussion will contribute will contribute to the unlocking of the immense potential of SP by:
• Identifying how different sectors, such as NHS, local councils and voluntary organisations, are working together and developing best practice
• Distinguishing the essential features of robust methods of evaluation and monitoring of SP schemes
• Exploring how local councils can harness the power of SP in helping to build healthy communities.
